MASS INTENTION REQUEST - Fill out this form to request a Mass intention. Mass Intentions are a great way to offer the greatest prayer, the Holy Sacrifice of the Mass, for someone who has passed away. Graces of the Mass are applied to that particular person’s soul, which speeds their journey toward God.

APPLICATION TO HOLY MATRIMONY - This application will get you started on your journey toward marriage in the Catholic Church. After filling out this form, someone from the parish office will contact you to further the process. You must apply no less than 6 months before your wedding date, but preferably you would apply around 1 year prior. It is necessary that you either be a member here at Saint John’s or have an immediate relative who is a member, in order for us to proceed with the request for the sacrament.

BAPTISM REGISTRATION FORM - Baptisms are celebrated on the 1st and 3rd Sunday of every month (unless otherwise stated). If this is your first child being baptized, a short introduction to Baptism course is required before the scheduled Baptism date. It is necessary that you either be a member here at Saint John’s or have an immediate relative who is a member, in order for us to proceed with the request for the sacrament.
